Is compounded semaglutide or tirzepatide legal right now?

Compounded semaglutide and tirzepatide are not FDA-approved, and the broad legal basis that made them widely available has closed. FDA declared the shortages resolved, which ended the shortage-based exception that let compounders supply them at scale. Large-scale compounding by 503B outsourcing facilities is now the subject of an FDA proposal to exclude semaglutide, tirzepatide and liraglutide from the 503B bulks list, on the basis that FDA identified no clinical need for it; the public comment period closed on July 30, 2026 and a final decision is pending. Narrow patient-specific (503A) compounding can still be lawful in limited circumstances, for example a documented allergy to an ingredient in the approved product, or a dose the approved product doesn't come in, but a compounded drug that is essentially a copy of an available FDA-approved product is not permitted. Whether any specific prescription is lawful depends on your prescriber, the pharmacy, and your state; this is not legal or medical advice.
